comment How do you scale a vector image to equal a measurement in inches, in illustrator CS3?
@Julie Select all the stroked paths and use Object > Expand Appearance to turn the strokes into shapes. Then increase the size. I suspect you would need to do this anyway. As joojaa says, NC equipment will follow the path and ignore the width of the stroke unless strokes are converted to shapes.

Great question! This is one of those things that's so nicely hidden, and so awesomely geeky, it can completely baffle anyone.

The Smart Object approach works well on your test file. Just remember to create the Smart Object, duplicate it, then apply the masks.

comment Is it possible to reference two anchor texts hyperlinks for the same paragraph in Indesign
I've expanded the answer. I suspect you're choosing "Paragraph" instead of "Text Anchor" in your workflow.

I've updated the answer. Still not entirely clear to me why you have the issue, because I can't reproduce it, but I've suggested other ways to approach this workflow that bypass the problem anyway.

Not unless you specifically export spreads (a handy way to do foldouts, which is what you're thinking of, but not the only case). I've not come across confusion with it, once it's understood. On the other hand, I'd also never put an even numbered folio on a recto, even in a pamphlet. :/

An alternative is to turn off "Allow Document Pages to Shuffle" after doing your Step Zero. This creates the same result.
