The Blend mode for the Pattern Overlay should be set to Overlay one of the others in that group, but it appears there is another problem at work here. Could you upload a copy of the file that is not displaying correctly, so I may take a closer look?

@Scott An alternative for you to consider is to proactively convert every major layer to a Smart Object before making your Layer Comps. This increases file size but should you ever need to add to or change a layer you can do so inside the Smart Object without losing the Comps. I don't like this "solution" because of the file size but if you are already saving multiple copies it's something to consider.

If no pixels extend beyond the canvas you should be able to select the small element, invert the selection, and then clear (delete). If this does what you want you can make an action and do it with a single click (Button Mode) or assign a keyboard shortcut. Do you mean something else?
